As you know, a group health plan was required to provide individuals covered by the plan with a notice of the plan's privacy practices by April 14, 2003, for a large group health plan and by April 14, 2004, for a small group health plan (a plan with insurance premiums or paid claims of $5 million or less). But did you know that the HIPAA privacy rules also require group health plans to provide covered individuals with a notice at least once every three years stating that the privacy notice is available and describing how to obtain it?
